Team List
Post this list of teammates in your company break room to show who has signed up and is on your team. Once people see the list of team members, they will want to join too.
Note to Captains: this is not a "sign up" sheet to join the team; it is a way to show who is already on the team—people still need to register online to participate in the event.

Doggie Dash Donation Form for Cash & Checks
If you collect donations in the form of cash or checks, you'll need to submit the donations to OHS along with the Doggie Dash Donation Form. Our staff will add these donations to your fundraising total. Download Doggie Dash Donation Form
